Question:  How do I find out who is processing may application or in charge of my case?

Answer:  If you turned in a child support application but have not heard back from anyone yet, call (303) 814-7145.

If your cash, food or medical application was already approved or denied, then you should have received a letter that tells you who processed the application and who is in charge of your case.  That name and phone number is on your letter in the upper right corner of the letter.  If you did not get a letter or are not sure, you can call us at (303) 688-4825 and enter extension 5341 for the receptionist who will research your question and tell you who you need to talk to.

If you mailed in an application or dropped off a cash, food or medical application and have not yet received a letter, you can call at (303) 688-4825 and enter extension 5341 for the receptionist who will research your question and tell you who you need to talk to.

If you already had an appointment for your cash, food or medical application, you should call the person that you met with.  If you do not remember who you met with, you can call us at (303) 688-4825 and enter extension 5341 for the receptionist who will research your question and tell you who you need to talk to.

If you had a cash, food or medical program in another county and don’t know who is in charge of your case, you can call us at (303) 688-4825 and enter extension 5341 for the receptionist who will research your question and tell you who you need to talk to.

If you are already getting child care assistance or have completed the interview and have a question about your case, call (303) 688-4825 extension 5340.

If you are receiving child care assistance in another county, you must have your case closed in the other county you are moving from, and reapply for benefits in the county you are moving to. Child care assistance does not transfer from one county to another.

Question:  How do I transfer my case?

Answer:  If you have an open child support case in another Colorado county or state, you need to tell your worker in your original county or state. 

If you lived in another county in Colorado and had an open cash, food or medical case, you can report your new address to us.  You can call the receptionist at (303) 688-4825 extension 5341 to report your new address.  The receptionist will not be able to tell you right away who your new technician will be.

If you lived in another county in Colorado and your cash, food or medical case closed, you will probably have to apply again.  You can call the receptionist at (303) 688-4825 extension 5341 to find out of your case closed and if you have to apply again. 

If you lived in another state and had a cash, food or medical case, it did not transfer to Colorado so you have to apply again (unless you had Supplemental Security Income).

If you had child care in another county, you must have your case closed in the other county.  Child care assistance does not transfer from county to county.  To get child care started in Douglas, call (303) 688-4825 extension 5340 to schedule and appointment to apply.

Question:  What will you need to process my application?

Answer:  The information we need to have for your application is different depending on the program.  Please read the list below for the program you have questions about.

For child support, you will have to give us:

  1. copies of each child’s birth certificate,
  2. copies of any court orders for child support you already have and a list of any payments you have already received,
  3. copy of your driver’s license or state identification,
  4. as much information as you know about the other parent, including for example his/her name, Social Security number, date of birth, address, and the name of his/her employer.  A photo is also requested if you have one, and
  5. your $20 application fee.  If you bring a check or money order, make it out to “Douglas County Human Services”.  If you bring cash, please have the exact amount because we cannot give you change back.

You may also be asked for:

  1. the paternity questionnaire if the father’s name is not on the child’s birth certificate,
  2. documentation of your current income (this includes for example, pay stubs, bank statements, unemployment, or Social Security benefits, and/or tax returns),
  3. documentation of any medical insurance premiums, day care costs, and/or medical bills for the children, and
  4. any court orders for parenting time.

For cash, food or medical programs, the information needed to process your application depends on what you applied for and your situation.  Please review this list and bring in all of the things that you have.  You should bring in the most recent information.  If we need additional information, we will let you know.  If you are not sure if something will be needed, it is better to bring it that information when you drop off your application or meet with the worker. 

  1. Identification and Citizenship – driver’s license, passport, alien card, naturalization certificate, birth certificate, court documents changing your name, Social Security card, health insurance cards
  2. Legal – divorce paperwork, custody documents, restraining orders, child support orders, eviction notice, Power of Attorney or Legal guardian paperwork
  3. Income – pay stubs from all current jobs, self-employment ledgers, information about pension/retirement, unemployment information, child support payments you are getting, veteran’s benefits, workers’ compensation payments, rent or room/board paid to you, student loans, gifts
  4. Expenses – rental agreement, mortgage statement, utility and phone bills, medical expenses, child care expenses, child support you are paying, promissory notes
  5. Immunizations – each child’s immunization records
  6. Resources – all vehicle/RV/boat/trailer registrations, bank and investment account statements (from banks, credit unions, savings, IRA, annuity, or stock/bonds) life insurance or burial plan information, other real estate/property

If you are applying for Medicaid, the Department will have to see the original identification and citizenship document (unless you are already getting SSI, SSDI or Medicare).

For child care, you will have to give us:

  1. Social Security cards for all family members
  2. Birth certificates for all children
  3. Picture identification for every adult in the household
  4. Immunization records for all children
  5. 3 months of pay stubs for each parent
  6. 3 months of child support verification

If you just started a new job, you have to give us a letter from your job.  The letter must include your hire date, rate of pay, how often you are paid, and your schedule. Also you have to give us pay stubs within 30 days from your application date.

If you are requesting child care while you attend school, you must provide an enrollment verification letter that includes your start and end date, the program you are enrolled in, the length of time it will take to complete the program, your anticipated salary upon graduation, anticipated graduation date, and daily schedule.  You must also provide a copy of all loans and grants that you are receiving.  We can only help with child care while you are in school if you can complete your program and earn your degree or professional license/vocational certificate within 2 years of the day you apply for assistance.  We cannot pay for child care for anyone who is working on a Masters degree or above. If you are in High School or in training to earn a GED, you can receive assistance until you complete your high school/GED education or you reach the age of 22.

Question:  How do I report changes to my case?

Answer:  There are a few ways you can report changes to your case:

  1. call us and speak with the person who is in charge of your case,
  2. fax the information to the person who is in charge of your case,
  3. drop off or mail the information to the person who is in charge of your case, or
  4. email the information to the person who is in charge of your case.

Changes need to be reported to your worker.

It is your responsibility to make sure that we have all of the most current information about your case.  Making sure that we have all the current information will get you the right benefit or payment and avoid any overpayments that have to be paid back.

If you are out of the state or country for 30 or more days, you need to tell your worker because this could affect your benefits.  

Question:  How do I get my Medicaid card?

Answer:  After your application is approved, you will get a Medicaid card for each person in the mail in about 2 weeks.  If you do not get the card after 2 weeks, you can call your worker.
