Declaration Forms

  • Commercial Personal Property Declaration Schedule
    Personal property includes machinery, equipment and other articles related to the business of a commercial or industrial operation. Taxpayers owning more than $50,000 in actual value of personal property per business are required to complete and return the Personal Property Declaration Schedule to the Assessor no later than April 15 of each year. Use this form for non-leased equipment.

    Personal property includes machinery, equipment and other articles related to the business of a commercial or industrial operation. Taxpayers owning more than $50,000 in actual value of personal property per business are required to complete and return the Personal Property Declaration Schedule to the Assessor no later than April 15 of each year. Use this form for leased equipment.

Appeal Forms

  • Personal Property Appeal Form
    Owners of business personal property who disagree with their valuation may file a protest during the statutory protest period. A protest form is attached to the Notice of Valuation mailed to personal property owners on June 15. An additional form is provided here for your convenience.
  • Abatement Forms 1 Year
    The abatement process is required to change tax amounts after the tax warrant is delivered to the Treasurer. Abatements must be filed within two years of the date the tax warrant was delivered (CRS 39-10-114(1)(a)(I)(A)). Use this form if only one year’s taxes are being abated.
  • Abatement Forms 2 Year
    The abatement process is required to change tax amounts after the tax warrant is delivered to the Treasurer. Abatements must be filed within two years of the date the tax warrant was delivered (CRS 39-10-114(1)(a)(I)(A)). Use this form if this is an abatement of two year’s taxes.
  • Agent Authorization
    If you are appealing property value but are not the owner of record, proof of the agent authorization is required. Please provide this form with any appeal documentation.
