Over the past year, the Douglas County Health Department (DCHD) engaged residents across the county to better understand their top public health concerns. Your input has now shaped action.
The 2025–2028 Public Health Improvement Plan (PHIP), informed by a Community Health Needs Assessment (CHNA), identifies key health priorities unique to our community and aligns local efforts with statewide strategies to ensure resources are used effectively and impact is maximized.
The CHNA analyzed a wide range of data sources—including direct input from Douglas County residents—to identify the most pressing public health challenges in the community.
The Assessment was supported by a series of public meetings hosted by AdventHealth in 2025, where residents shared feedback and ideas for improving public health outcomes in Douglas County.
Residents identified mental health and reliable access to essential needs and healthcare as top concerns. In response, DCHD developed targeted action plans to address both areas.
To support mental health, DCHD will expand peer support services, increase awareness of available mental health resources, and implement additional strategies aimed at prevention and early intervention.
To improve access to essential needs, DCHD plans to diversify communication methods for transportation and support services, identify and strengthen outreach partnerships, and prioritize underserved areas, including rural communities within Douglas County, among other strategies.
The PHIP aligns strategically with the Colorado State Health Improvement Plan (SHIP), the Community Health Needs Assessments, and Douglas County’s broader strategic priorities, ensuring coordination across healthcare systems and public health efforts.
By integrating state and regional frameworks with local data and community input, the Plan establishes a coordinated approach to addressing health priorities and maximizing the impact of available resources for Douglas County residents.
